2011 YLS SUMMER 70 LACROSSE CAMPS
SESSION 1 Grades K-2: $95 fee, JUNE 1, 2011 deadline
SESSION 2 Grades 3-5 & 6-8: $125 fee, JUNE 1, 2011 deadline ($200 total if doing Session 2 & 4)
SESSION 3 Grades 9-12: $150 fee, June 20, 2011 deadline
SESSION 4 Grades 3-5 & 6-8: $125 fee, June 20, 2011 deadline ($200 total if doing Sessions 2 & 4)

REGISTRATION IS OPEN: Payment is due on or before the posted deadline.
IMPORTANT: EACH CAMP HAS A ROSTER LIMIT. ROSTERS ARE DETERMINED BY THE ORDER OF THE PAYMENTS RECEIVED PROVIDED THAT THE REGISTRATION INFORMATION IS ALSO COMPLETE.
After payment deadlines, waiting list players will be offered open positions in the order the online registration is received. If you are notified that you are on a waiting list, please do not send in a payment until you are notified that the camp position has been offered to your child.

There are no prorates. There is a $50 reimbursement up to the day prior to the 1st camp session date, and no reimbursements on or after the 1st camp session date.

Amateur Athletic Minor Waiver and Release of Liability

In consideration of being allowed to participate in The Youth Lacrosse System, L.L.C.:

We, the undersigned, for ourselves, our heirs, executors and administrators, waiver and release and forever discharge The Youth Lacrosse System, L.L.C., its staff, officers, agents, representatives, employees, successors and assigns of and from any and all rights and claims for bodily injuries and damages to personal property which may be sustained or occur during participation in camp activities, or from camp, whether paid damages, injury or loss are due to negligence or not.

I/We certify that the applicant is in good physical and mental condition to take part in
The Youth Lacrosse System, L.L.C., and we have notified the YLS Directors of any pre-existing conditions.

Please make sure you have filled in all information before submitting your registration.

NO registration is complete if the Amateur Athletic Minor Waiver is not checked off.

You will receive a confirmation email from The Youth Lacrosse System, L.L.C. when we have received your payment.
